Abstract	:	Background: Elderly patients transferred from a long-term care facility (LTCF) contribute to overcrowding in the emergency department (ED). When seen in the ED, they have longer hospital stays, require comprehensive testing to establish a diagnosis and use unnecessary resources due to lack of communication between facilities. One of the most challenging aspects in caring for LTCF residents is a lack of communication about a resident’s health status, medications, and history upon transfer.
