When dealing with aneurysms, the initial subarachnoid hemorrhage can be devastating - creating difficulties in management - and cerebral vasospasm still has to be solved. The hemodynamic consequences of the occlusion of large AVMs are difficult to predict and combined or alternative treatments may be preferable to surgical removal of these lesions. For less common malformations - such as dural fistulas, cavernomas and venous angiomas - the indications for treatment have still to be established.
