Abstract : The second edition of this book was necessary in order to accommodate the rapid rise of interest in this subject. It provides a concise summary of these developments from the bench to the bedside. At stake is the enormous importance of the issue at hand: how to select the patient with severe left ventricular dysfunction who may benefit from coronary revascularization.
